BACKGROUND AND OBJECTIVES: The use of opioids as first and second line agents to adequately treat pain requires systematization in different clinical syndromes which course with acute pain. This study aimed at discussing recommendations for the use of opioids in acute postoperative pain, neuropathic pain, musculoskeletal pain and pain during gestation and lactation. CONTENTS: This review has addressed the use of opioids in frequent chronic and acute painful syndromes, in gestation and lactation, discussing indications, drugs used, doses, risks, complications and recommendations. CONCLUSION: Opioids for acute postoperative pain have been broadly studied and are established for minor medium and major surgeries. Recommendations for the use of opioids in neuropathic and musculoskeletal pain are restricted to second line treatment and require further discussions. Few studies have investigated the interaction of opioids with physiologic changes typical of gestation and the repercussions of the use of such agents to treat acute and chronic pain in the short and long term.

BACKGROUND AND OBJECTIVES: Several human studies have shown an inverse relation between pain perception and blood pressure. This study aimed at investigating the association between musculoskeletal pain report and hypertension in a group of workers. METHODS: Using a body diagram with image and intensity scale (1 to 10), 349 workers (243 males and 106 females) were asked about the presence and sensitivity to musculoskeletal pain. All were submitted to blood pressure measurement and diagnosis of hypertension by the occupational physician. RESULTS: One hundred workers (28.7%) have reported some type of musculoskeletal pain and from these 12 were hypertensive and 88 normotensive. There has been no difference in musculoskeletal pain prevalence and sensitivity between hypertensive and normotensive workers. Notwithstanding the lack of significant difference, in average hypertensive workers had higher prevalence (38.7% vs 27.7%) and sensitivity to pain as compared to normotensive workers (2.3±0.8 vs 2.1±0.9, respectively). CONCLUSION: It was not possible to confirm in the group of studied workers literature evidences that hypertensive individuals have lower pain prevalence and sensitivity as compared to normotensive individuals.

Childhood obesity increases susceptibility to musculoskeletal injuries. The purpose of this study was to describe the prevalence of overweight and obesity and to identify differences in posture and musculoskeletal pain among eutrophic, overweight, and obese students. Participants were 420 students, 252 (60%) were females and 168 males (40%), with a mean age of 11.1 (±2.3) years. The posture of all participants was qualitatively assessed; the quantitative postural evaluation was performed using the Postural Assessment Software (PAS/SAPo) for a population subsample of 99 participants. An adapted version of the Nordic Musculoskeletal Questionnaire was used for pain assessment. Data were analyzed descriptively and via statistical tests (significance level of p<0.05). The target population exhibited 22.1% of overweight individuals and 14.1% of obese. Compared to the eutrophic students, the postural evaluation showed a higher knee valgus angle, higher incidence of thoracic kyphosis, and greater prevalence of lumbar hyperlordosis in overweight and obese students (p≤0.05). No association between overweight and pain complaints was detected (p=0.994).

The Nordic Musculoskeletal Questionnaire (NMQ) quantifies musculoskeletal pain and activity prevention in 9 body regions. The purpose of this study was to develop an extended NMQ (NMQ-E) to collect greater information regarding musculoskeletal pain, examine test–retest reliability and the reproducibility of alternate administration methods. Reliability was examined using observed proportion of agreement for all (Po), positive (Ppos) and negative (Pneg) responses, kappa (κ), proportion of maximum kappa achieved (κ/κmax), intra-class correlation coefficient (ICC) and standard error of measurement (SEM). The NMQ-E was self-administered by 59 Bachelor of Nursing students at a 24-h interval with mean Po = 0.88–0.98 and κ/κmax = 0.71–0.96 for 10 dichotomous questions and mean ICC(2,1) = 0.97 and SEM = 1.05 years for the age at symptom onset question. The NMQ-E was completed via self and interview administration by 31 student nurses at a 0.97 ± 1.14 day interval with mean Po = 0.92–0.98 and κ/κmax = 0.76–1.00 for binary questions and mean ICC(2,1) = 0.90 and SEM = 1.51 years for age at symptom onset data. In both sub-studies, mean Ppos was lower than mean Pneg and low prevalence reduced κ in many instances. The NMQ-E collects reliable information regarding the onset...

BACKGROUND Chronic musculoskeletal pain is common and has been increasingly managed by opioid medications, of which the long-term efficacy is unknown. Furthermore, there is evidence that long-term use of opioids is associated with reduced pain control, declining physical function and quality of life, and could hinder the goals of integrated pain management. Electroacupuncture (EA) has been shown to be effective in reducing postoperative opioid consumption. Limited evidence suggests that acupuncture could assist patients with chronic pain to reduce their requirements for opioids. The proposed research aims to assess if EA is an effective adjunct therapy to standard pain and medication management in reducing opioids use by patients with chronic musculoskeletal pain. METHODS In this multicentre, randomised, sham-acupuncture controlled, three-arm clinical trial, 316 patients regularly taking opioids for pain control and meeting the defined selection criteria will be recruited from pain management centres and clinics of primary care providers in Victoria, Australia. After a four-week run-in period, the participants are randomly assigned to one of three treatment groups to receive EA, sham EA or no-EA with a ratio of 2:1:1. All participants receive routine pain medication management delivered and supervised by the trial medical doctors. Twelve sessions of semi-structured EA or sham EA treatment are delivered over 10 weeks. Upon completion of the acupuncture treatment period...

INTRODUCTION: There are significant human, social, and business costs associated with work-related musculoskeletal disorders (WRMSD), despite being the subject of extensive research and widespread interventions. A profile of WRMSD can be obtained via workers compensation data, but the distribution of precursor conditions, such as localised body pain and discomfort, are not routinely assessed. Moreover, there is a paucity of data in Australia. The experience of pain has a psychological component, and it is now believed that psychosocial factors are an independent cause of WRMSD. As a result, psychosocial approaches are increasingly applied to WRMSD prevention. In particular, the Trans-Theoretical model has recently found application in workplaces, and there is some evidence in a UK study of benefit in tailoring interventions using a stage of change (SOC) construct. Here, the interventions are designed to address workers readiness to change behaviour. AIMS AND OBJECTIVES: The research in this thesis aimed to describe the relationship between psychosocial factors and musculoskeletal pain and discomfort (MSPD) experience in workplace settings, and to evaluate the effectiveness and implementation of a stage of change approach to MSPD prevention. The objectives of the research were as follows: 1. Determine the prevalence of MSPD in a sample of South Australian workplaces. 2. Examine relationships between MSPD and a range of individual...

AbstractIntroduction The epidemiological profile is considered a sensitive indicator of living conditions and the disease process. The musculoskeletal disorders are the most common causes of pain and can lead to disability or restriction of daily activities. These disorders take the name of RCT/OWRD when they are associated with work activities and may be associated with risk conditions at work. Objectives To describe the epidemiological profile of supermarket workers with musculoskeletal disorders under treatment at a physiotherapy clinic in the city of Porto Alegre, Brazil. Methods This was a cross-sectional, observational study which used retrospective data collected from 360 records of patients treated from January 2010 to December 2011 in a physiotherapy clinic that provides health services for a supermarket chain. Results There was a predominance of females (73.9%), aged 30–39 years (35.1%); 63.0% reported being single and 73.4% lived in Porto Alegre. The most commonly reported occupation was cashier (31.2%). The main reasons for referral to physiotherapy treatment were low back pain (21.4%), neck pain (19.7%), pain (16.1%), subacromial bursitis (13.9%) and back pain (12.2%). Among the signs and symptoms 95.8% of the sample reported pain in chronic phase. Conclusion The prevalence of musculoskeletal pain was high in this group. The presence of pain can disable the worker for daily activities and physiotherapy becomes the therapeutic procedure of choice for their rehabilitation.
